Add issue templates

Add issue templates to avoid invalid issues. (There's no useful information in #393, #388, #395, and many other issues).
Adding an issue template with the following fields would help:

• Minecraft Version [e.g. 1.17.1]
• Bug Description [e.g. Game crashing when...]
• How to reproduce? [e.g. Click on "..."; Launch the game; etc.]
• Logs/Stacktrace (maybe not direct upload, but GitHub Gist)

I suggest adding the following checklist too:

• I'm using the latest Fabric/Fabric-API version for my Minecraft version
• I have downloaded and put into the 'mods' folder the latest OptiFine for my Minecraft version.
• I'm using the latest OptiFabric version for my Minecraft version.
• I'm not using the Iris or Sodium.

There's a good PR with this addition.

Note: "using the latest Fabric version" is currently the cause of an error for a lot of people. The mod isn't compatible with Fabric 0.12.5 (0.12.x?), and only works at maximum 0.11.x. This is noted in the error, but many people can't read errors easily.
