When importing an exported data file, most questions are not displayed on the map

When I upload data from a previous session (same version), most questions in the imported data are not displayed on the map. For example, I uploaded a test JSON with data from 127 questions, and (after a few-seconds delay that didn't exist prior to the most recent PR), only 12 were displayed.

• These do not appear to be the first 12 questions I answered in the session from which I exported the JSON.
• The computed knowledge values shown on the map do appear to reflect all exported questions.
• Unlike previous times I've encountered this issue, answering additional questions does not cause the rest of the questions to appear
• Also unlike previous versions of this bug, it happens when using the upload button on both the landing page and map page

Fixed: Import now displays all question dots on the map.

Root cause: responsesToAnsweredDots() skipped any dot whose question wasn't in the questionIndex yet. During import (especially from landing page), the index isn't fully populated.

Fix: Allow dots that already have x/y coordinates to render even without a questionIndex entry. Commit cd1d0d6 on 004-persona-user-testing.
