Merge gridappsd-hackathon and gridappsd-training repos

Need to merge https://github.com/GRIDAPPSD/gridappsd-hackathon with https://github.com/GRIDAPPSD/gridappsd-training

All documentation issues and work tracking under hackathon repo
All new code and source for RTD under training repo. 

Also have been assigned high-performance server by RTD, so prefer to keep training repo as is and merge hackathon into it.
