Add columns to the service provider spreadsheet

We're going to need these columns in the doctors Google sheet. Since SSI said they want the app to be more like a health engine and not just a find doctors app. So we'll need to collect more data. We're going to need to add these columns to the spreadsheet on who/where doctors are.

• Service type (doctor, pathology, service provider, pathologist)
• Hours open
• Days open
• Needs appointment or not

Hi @equivalentideas have you read my messages? No urgency etc, but SSI requested some changes, so I've added these columns to the data form.
I've requested they start to fill in some data so we can have some data to test the app on and then implement new features.
Then we can provide the text to translate to them, when we know what features we need.
Does this sound like a good plan?
