this for example is GPT5-codex medium using kilocode provider

Reproduction steps

1. Create any file (in my case it was a single livewire file)
2. prompt: read this file @/app/Livewire/Counter.php and use context7 to get the latest livewire 3.x docs
3. wait for the output

I'll add another point. When I set a task and write "use FileSystem MCP," in ~20-40% of cases I see that the kilocode calls FileSystem MCP without specifying any required arguments (even commands like read, write, or anything else).

I've noticed that if you set a task that isn't just "use FileSystem MCP," but "use FileSystem MCP—the rules for how to call it are in your instructions" —errors are less common.
