feat: update talent costs and character sheet

- Add an explicit Cost column to all talent tables in the Core Rulebook.
- Rework the Bleeding status effect to apply a flat -2 Defense penalty instead of stacking damage.
- Restructure the character sheet layout by moving Journal and Conditions to Page 1.
- Relocate and significantly expand the Skills list on Page 2 using a new grid layout.
