View Patients Page: Excessively Long Inputs will cause horizontal overflow

To reproduce:

1. When creating a patient, type a really long name
2. When displaying the admin details, horizontal overflow seen

To Fix:
Probably need to set a max width for the table Modal, and for its individual columns
