Remove feature flag

Issue Description

Remove the feature flag from the multiple extension request. Also, fix the UI where the progress bar stretches out of the card.

Expected Behavior

Multiple extension request forms should be accessible to the user without a feature flag. And the progress bar goes out of the card container.

Current Behavior

The user has to add a feature flag to raise multiple extension requests. UI looks bad because of the progress bar.
