Have the ability to manually add observations to a target page

We would like for users to be able to manually add observations from any telescope (planned, completed and pending) to a target page.

This is something that was brought up for 3I coordination, and there was a resulting form to spreadsheet that is now on a website. It would be great if our TOM could be a hub for this kind of coordination in the future.
