Frequent 500 error - InternalFailure: UnknownError

Not sure how to reproduce, but when using these samples:

speech-to-speech/sample-codes/websocket-nodejs
speech-to-speech/repeatable-patterns/bedrock-knowledge-base
```

I am frequently getting `InternalFailure: UnknownError` see screenshot of server log:

Any tips on how to troubleshoot?
