Enforce phone number length and format

On User Profile, we continue to get phone numbers that do not conform to field length and format (the phone number repeated multiple times within the field). Request to debug and enforce field length.

This would apply to:
Home phone
Mobile phone
Other phone

in all places where they can be entered or edited on original UP submission, MyProfile edits in MyConnect, and SMDB interface.
