Use checkboxes for "how did you hear about us"

Information on how people learned about us can be really high value when checked over time. The current free-form is a little demanding, and people will mostly not answer in a useful way that can be tracked over time against our outreach efforts :)

We should add checkboxes to make it simpler, and an other field.
