Improve quote detection in complex html situations

Something like `"<em>hi there"</em>` currently totally breaks quote detection further down the paragraph. This issue's pretty technically tricky; I'm mostly filing it so that if I have ideas I have a place to put them.

One alternative would be to add an ability for users to mark such a section to come back later, or remove/insert misidentified buttons via selection somehow
