Input group breaking changes

There has been breaking changes since beta 3 regarding input groups.

It's using the old input group button styling which breaks on current version of bootstrap.

From the link: We’ve dropped .input-group-addon and .input-group-btn for two new classes, .input-group-prepend and .input-group-append. Using input-group-btn, as this plugin currently does, no longer styles the input correctly.
