Civil Rights Information

The Civil Rights Information / Permission Slip needs to be filled out by a parent after the Medical Information form.

1. Add this to the DB (some may already be handled)
2. Generate a notification to deal with this on the parent dashboard.
3. Add a new column to the admin's Manage Student grocerycrud where they can view and modify this form.

NOTE: parents don't need to be able to modify this
