Create CONTRIBUTING and PULL_REQUEST_TEMPLATE files

Consider moving material from the README to .github/CONTRIBUTING.

Also consider creating .github/PULL_REQUEST_TEMPLATE that says to run yarn lint and (once it exists) yarn test.
