Support handling multiple paths

Why does the module only alter the first path rather than all of them?
I suggest to add that feature if it complies with the project, since most of the time several scripts are piped through plugins and then concatenated,

Do you have a sample scenario, where this could be useful? Is it not more convenient to prepare a debugging configuration with separate files?

I added a small example how to use it with separate source and built files and i use it with a bigger project too.

If used in web applications such as Laravel it can often be convinient to glob and concat source files.

I wrote a sample project structure that uses vendor and project .js files aswell as some coffeescript classes and code.

At the moment relative source files work in some cases and only on the first globbed file.
In actual web applications that use webpack, babel, etc. things could even get far more complicated.

I managed to break it down to 2 issues which prevent the above scenario from working as expected.

Apparently it does not work correctly with gulp-concat (even if used to alter paths before the actual concat, since it accounts for subdirs).

Also it would have to reevaluate paths if loadMaps: true option is used on gulp-sourcemaps.

I finally got to this issue myself. Supporting output files concatenated from multiple source files went surprisingly easy by changing the way of computing paths relative to the output directory. (Providing all source files are located below file.base, usually some "src" directory.)

Output produced by gulp-concat was easier to handle, because no modules have to be recognised. Just all source paths in the sourceMap array have to be updated. Yes, loadMaps: true was necessary to preserve the very original sources. As soon as the multiple sources depended on each other, a module bundler was needed. I used rollup, but the principle is the same with browserify or webpack. The link to your sample project got broken. I added my simple example.

I added the support for multiple files in a source map in version 1.1.0.
