DSF extraction in one file

Add an option du extract DSF material in one file natively from the ISO file (CUE sheet creation selected by default).
Will be very useful for listening concerts with a reader unable to make gapless with dsf files, for instance, and avoid to use another tool to concatened individual files (without knowing what it does).
