Improve support for Windows

• Tests for ssh command on Windows client
• Tests for sftp server command on Windows client (may need to change the default command)
• Tests for ssh server/ssd (in particular, the default shell which will be cmd.exe if the user hasn't changed it)
• Improve docs
• Provide link to OpenSSH installation and setup docs in readme (also explains changing the default shell)
