Respect .gitignore #23

We're having a slight problem where micode will attempt to add the contents of the ./thoughts directory to the git repo, even though ./thoughts is in .gitignore. It will force it, even though it is ignored.

I admit, we aren't sure if this is something specific to micode, the backend LLM, or OpenCode. We suspect micode because one of the guys said he saw a thinking message which said to commit the plan.

I wish I had more details to share, but if you could take a look we would appreciate it. It keeps cropping up, and we don't want the thoughts directory as part of the repo.
