Responsive theme

It would be great to make this theme responsive, or at least allow the testing of responsive styles.

We've been using this (great) theme and here are my quick 'n dirty additions to make this theme responsive:

• holograph.css
• with some changes to swap around the title and nav in our header

Ideally this theme would be implemented mobile-first but I hope my above version might be a useful reference. Some alterations to the JS might be desired to restrict the stick menu to larger viewports.
