Fixes #4522 and adds client-level terminal focus tracking.
This makes focus behavior consistent across tabs, panes, fullscreen, floating panes,
and multiple clients.
I have traded any urge to prematurely optimize in favor of clarity and resilience to future changes. I also doubt that the overhead introduced is in any way significant.
