Data Deidentification Process and Codes

TODO:

1. Update readmes with deidentification steps
2. Hash identifiers in the meta + notes files
3. Match the meta and notes
4. Include Image accession numbers as supertable columns
5. Clean up and push the code for EMR deidentification
6. Clean up and push the code for Supertable deidentification i. De-identify by serializing the index ii. Patient matching file will have the CSN -> deidentified CSN -> time. Make sure we have the deidentified csn -> time mapping on PACE.
