What does this PR do?

This PR adds the missing app directory to the Clerk starter template. This is essential for the project to function correctly within the new app directory structure used in modern Next.js applications.
Update readme section.

Test Plan

Verified that the project builds successfully after adding the app directory.
Checked that routing and basic functionality work as expected in a local environment.
