Added "Terms of Service" Dialog as mentioned in issue #8

Using jQuery UI Dialog Widget:

• A dialog is displayed on loading
• Replaced jquery-ui files to include the dialog widget's files
• Got the text from hand-o-matic terms of service

Thanks @andreasbastian. I really love this project!
I have some suggestions on the development which I would like to aggregate in an issue and start working on them.
It will include all the requests posted here along with a suggested development path.

I just got back from a week in Haiti working with one of our partner clinics and have a bunch of new functionality/features to document to enable support of a new type that they will be using.
