update remaining fields phenopackets

What's new

• add remaining phenopackets fields
• some fields are not finished until we figure out how to group them

How to test

• pull this branch for candig-api
• pull Update omop-sample.json CanDIGv3#32 for candigv3
• rebuild
• ingest omop-sample.json
• call to see the expected result

Introduced a mapping file that we can continue to put concept mappings into and made some improvements
tried to abstract some methods a bit more
grouped treatment intent and response on medical action by respective episodes
